General Prerequisite / Criteria

• The form should be completed in 3 copies and must be typed.
• Each employer must fill in a different address endorsement form for each of the company’s activities in accordance to the reference of PJB.
• Employer must include a valid place rental agreement.
• Place of activity must be in a Building/Commercial area.
• Must have a special condition of the land (Sideline Business) if a business activity is on a private land.
• Business activities should not be located at the National Housing Scheme/Skin Tanah Kurnia Rakyat Jati and Water Village.
  
• If there is any additional business address, office or branch, the employer must acknowledge and obtain approval from the Department of Labour by submitting this address endorsement form.
  
• The employer must acknowledge and obtain approval from the Labour Department every 2 years by filling in this address endorsement form.
  
• This form can be submi.tted by the employer or registered employment Agency.

• Incomplete application form, not attaching required documents and using edited form will not be accepted.
• Form must be submitted before applying Foreign Workers Licence (LPA).
• If one of the addresses is not valid then the Foreign Worker License (LPA) will be cancelled immediately.
