How to use the Service

• Form can be downloaded here.
  
• Completed form should be submitted to Ground Floor, Counter Services, Labour Department.
  
• Documents Required :
  
• Original copy of Domestic Helper License
  
• Valid copies of the applicant's and his / her spouse's identity cards
  
• Copies of the applicant's latest salary slips
  
• Completed financial details form (BUR300)
  
• Letter explaining reason's for additional application
  
• Copy of authorization letter for pensioners
  
• A copy of identity card for family members where applicable (Special Approval)
  
• Copies of spouse's latest salary slips. (If needed to support application)
  
• A copy of birth certificate and / or identity card for children or other family members living in the same household
  
• 2 copies of Exemption Form (if applicable)
  

• Additional Documents for Foreign Citizen :
  

• A copy of the Contract of Service or offer letter from the Government of His Majesty the Sultan and Yang Di-Pertuan of Brunei Darussalam
  
• A copy of the Contract of Service between the applicant and the employer endorsed by the Commisioner of Labour
  
• Approved Work Pass Recommendation (BUR500/555) by the Department of Labour.
